This is an online questionnaire designed to enable collection of data for a major World Health Organisation study, sponsored by GSK, on the Quality of Life of patients visiting GP surgeries. The user interface is written in Flash and Visual Basic and patient information is written to a database. A second program, the viewer, analyses the data and allows the GP immediate access to the results in a variety of formats.
